A whistleblower can submit information:
- digitally by using the following websites:
- http://www.cftc.gov/
- http://www.whistleblower.gov/
- in writing by completing Form TCR and mailing or faxing the form to the CFTC

In order to be eligible for an award in connection with the whistleblowing the whistleblower must declare under penalty of perjury that the information submitted is ture and correct to the best of their knowledge and belief.
The whistleblower’s identity must be disclosed and verified prior to the payment of the award.
