Survey: States diverge on Medicaid work requirement plans

With Medicaid work requirements set to take effect on January 1, 2027, a survey reveals growing concerns and wide variation in how states are preparing to implement them.

On July 4, 2025, President Trump signed the so-called One Big Beautiful Bill Act (OBBBA), which includes a federal mandate to implement work requirements for the adult Medicaid expansion group.

Medicaid work requirements under the law will take effect January 1, 2027, requiring adults in the Patient Protection and Affordable Care Act (ACA) expansion
